Full Time Care Manager Lead - Hematology/Oncology

Private practice in Ann Arbor is seeking a
full time Care Manager Lead - Hematology/Oncology.

Description:
This position is responsible for operationalizing the Care Manager Model across multiple IHA Primary Care and Specialty practices. This work is done under the direction of the Associate Director of Care Management. The Care Manager Lead will have supervisory responsibility for multiple Care Managers. The main responsibilities will include the development of clinical standards for care management; development and coordination of training; competency assessment; staffing/ productivity tracking; and mentoring of this team to support project deliverables. The Care Manager Lead supports the integration of the care management staff within the office team through process improvement, workflow redesign and training. This position serves in an expanded health care role to collaborate with specialists, community agencies, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services. Assesses plans, implements, coordinates, monitors and evaluates the aggregate work of the care management team to assure core project deliverables are met. Collaborates on the development of an infrastructure that assures that systems are in place that optimize the patient's health status; Integrates evidence-based clinical guidelines, preventive guidelines, and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care. This position will support IHA practices for all PCMH-related issues and will assist practices as needed to ensure their PCMH readiness for the re-designation process.

Qualifications:

  • Education
    • Bachelor of Science in Nursing or equivalent combination of education and experience
    • In addition, prefer completion of self-management support training
  • Credentials/Licensure
    • Valid, unrestricted RN license in the State of Michigan
    • CCM Certification is required within 6 months of taking on this role, if not already acquired
    • Valid CPR certification
    • CCM/CCP certification preferred
  • Minimun Experience
    • 5 years nursing experience
    • Working knowledge of chronic disease management
    • Experience with adult, pediatric and specialty patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting
    • Previous care management experience, experience as a participant in continuous quality improvement, completion of self-management support training and previous supervisory/management experience all preferred
