Palliative Care Program – Partnership with Carelon
Health Alliance Plan by Henry Ford Health partners with Carelon— a provider of non-hospice, community based specialty palliative care services. The Carelon Palliative Care at Home program offers a personalized and comprehensive approach to support patients at any stage of a serious health condition. Palliative care is offered based on need rather than prognosis. It aims to improve quality of life rather than serve as end-of-life care. It does not necessarily lead to hospice care. READ MORE
