Policy Change

Effective May 1, 2026, we’ll no longer require precertification for Medicare fos:

Fulphila® (pegfilgrastim-jmdb) (Q5108)

Changes to commercial drug lists (found in OfficeLink below)

On July 1, 2026, we’ll update our pharmacy drug lists. These changes may include things like:

• Adding or removing specialty, brand-name and generic drugs
• Changing preferred or non-preferred drugs
• Changing prior authorization (PA), quantity limits and step therapy requirements

You’ll be able to view the changes as early as May 1, 2026. They’ll be on our Formularies and Pharmacy Clinical Policy Bulletins page.
